View Policies & Forms

In Policies & Procedures section
1.	Click on the Policy document link. The document will open up in a pop up window.
	or
2.	Right click on the link, click Save Target As... and save this file to your system. Then and open this saved file and view it.
In Forms section
	Notification Form and Waiver Request Form. Either click on the link and save the opened form or right click on the link and click Save as Target and save the file to your system.
	You can take a look at the form before going ahead with the online submission. If do not want to submit Notifications or Waiver Requests online, you can take a copy of this forms provided here, fill in the details and mail it to:
